You can use this form to request spoken or sign language interpretation and/or translation, an assisted listening system, Braille materials, post-meeting recordings, and/or special transportation for any public meeting organized by the City. If you have questions, please contact the City Clerk's Office at (303) 335-4519 or email clerksoffice@louisvilleco.gov.

This form should be submitted at least two (2) business days prior to the public meeting organized by the City.

Use este formulario para pedir un intérprete o traductor de Español o de languaje de señas, un sistema de asistencia auditiva, soporte Braille, grabaciones despues de la reunion y/o transporte especial para cualquier reunión pública organizada por la Ciudad. Si tiene preguntas, llame a la oficina del City Clerk al 303.335.4519 ó mande un correo a clerksoffice@louisvilleco.gov.
