Program Cancellation Form

The following charges will be assessed for cancellations. All programs require refunds to be requested 7 days before classes start. There is a $10 service charge per class for cancellations with a 7-day notice. Cancellations after that time will result in a 100% CLASS FEE SURCHARGE. Programs canceled by the Recreation & Senior Center because of low enrollment will receive a full refund or account credit.

Summer Adventure Camp, Preschool, and Youth Sports have separate cancellation policies. Please check the current catalog for details.
